RESUMO

Glenohumeral joint is the most frequently dislocated joint of the human body. Concomitant fractures of the coracoid process and tuberculum majus in humeral dislocation of the shoulder joint are rarely described. Concomitant fractures are results of a significant contraction of the surrounding muscles and rotator cuff during a cerebral paroxysm. Due to the small number of cases, the treatment of such injuries is not simple and it is based on an algorithm for treatment of isolated injuries of these anatomical structures. In this case report, we describe a concomitant fracture of the coracoid process and tuberculum majus during an anterior shoulder dislocation in 25-year-old patient after an epileptic seizure. The injury was treated in our department surgically, with a good functional result. The absolute Constant score for the operated arm is 95 points, the relative Constant score is 97%, DASH score 0, VAS score 0.

OBJECTIVES: Our objective was to assess the accuracy of radiation-free magnetic method of scoliosis curve measurement (Ortelius 800) and its usefulness in clinical practice, as well as to judge whether it can replace x-rays, which represent currently the golden standard for scoliosis measurement. BACKGROUND: Patients with adolescent idiopathic scoliosis have to undergo multiple full-length spinal x-rays. Repeated exposure to ionizing radiation leads to higher incidence of breast cancer, thyroid cancer, and leukemia. One of the radiation-free methods that are candidates for replacing the x-rays is the magnetic measurement of the spine with Ortelius 800. METHODS: Twenty-eight patients with adolescent idiopathic scoliosis were measured with Ortelius. Data from Ortelius were compared with standard spinal x-rays taken the same week. RESULTS: The average Cobb's angle measured by x-ray was 35.11 degrees. The average absolute difference between Ortelius and x-rays was 6.69 degrees. The difference was statistically significant (p=0.0443). An acceptable difference was measured in subgroup of curves that do not exceed 40 degrees (difference 5, p=0.8915). CONCLUSION: Ortelius is most accurate in curves in range of 15-30 degrees of Cobb's angle. In larger curves it is increasingly inaccurate. Most important for clinical practice is the Cobb's angle range over 40 degrees, where the conservative therapy is to be replaced with surgery. Ortelius is not accurate enough in this range and therefore cannot fully substitute full spinal x-rays. It can be used to reduce the number of x-rays in the follow-up of smaller curve angles (Tab. 4, Fig. 8, Ref. 10).

OBJECTIVE: The purpose of this study is to review bacterial isolates from cases of urinary tract infection (UTI) and their antimicrobial susceptibility pattern for the years 2005-2007 in St Kitts. It is hoped that the study will be of use in the treatment of cases of UTI in St Kitts. METHODS: The laboratory records at St Francis Hospital, Basseterre, St Kitts, for bacterial isolates from cases of urinary tract infection and their susceptibility profiles for three years, 2005-2007, were retrospectively reviewed and compared. RESULTS: A total of 595 isolates of 13 species of pathogenic bacteria were recovered from cases of UTI. Escherichia coli was the predominant species recovered each year. Among the other species frequently recovered were Citrobacter spp, Enterobacter spp, Klebsiella pneumoniae and Pseudomonas aeruginosa. CONCLUSION: This study, the first of its kind from St Kitts serves to emphasize that treatment of UTI should be instituted generally on the basis of antimicrobial susceptibility tests.


OBJETIVO: El propósito de este estudio es examinar los aislados bacterianos de casos de infección del tracto urinario (ITU) y su modelo de susceptibilidad antimicrobiana durante los años 2005-2007 en Saint Kitts. Se espera que el estudio sea de utilidad en el tratamiento de casos de ITU en Saint Kitts. MÉTODOS: Los archivos de laboratorio del Hospital Saint Francis, Basseterre, Saint Kitts, sobre los aislados bacterianos de los casos de infección del tracto urinario y sus perfiles de susceptibilidad durante tres años, 2005-2007, fueron examinados y comparados retrospectivamente. RESULTADOS: Un total de 595 aislados de 13 especies de bacterias patógenas fueron recuperadas de casos de ITU. Escherichia coli fue la especie predominante recuperada cada año. Entre las otras especies frecuentemente recuperadas se hallan: Citrobacter spp, Enterobacter spp, Klebsiella pneumoniae y Pseudomonas aeruginosa. CONCLUSIÓN: Este estudio - el primero de su tipo en Saint Kitts - sirve para enfatizar que el tratamiento de la ITU generalmente debe establecerse sobre la base de pruebas de susceptibilidad antimicrobiana.
